If you have a suggested patriot name or a relative in DAR, proceed

directly to the Worksheet.

Please submit your completed worksheet, accompanied by vital records for generations 1-3; i.e., long form birth certificates, death certificates if applicable, and proof of marriage. If your lineage connects in generation 2 or 3 with a family member in your line, you may not be required to furnish information for that generation or others already established.
